Psychiatrists and clinicians use DSM-IV-TR to diagnose illness or disorders. A DSM-IV-TR diagnosis is divided into five parts and each part is known as an Axis. Every Axis gives information on different types of disorders, such as;
Axis I
In Axis I it handles clinical disorders, learning disorder and mental disorders.  Some common Axis I disorders include psychotic disorders, depression, anxiety disorders, bipolar disorder, mood disorders, ADHD, schizophrenia, bulimia nervosa, anorexia nervosa, and autism spectrum disorders.
Axis II
Axis II deals with personality disorders and mental retardation. The Common Axis II disorders include personality disorders: schizotypal personality disorder, paranoid personality disorder, avoidant personality disorder, schizoid personality disorder, borderline personality disorder, histrionic personality disorder, antisocial personality disorder, narcissistic personality disorder, dependent personality disorder, obsessive-compulsive personality disorder; and intellectual disabilities.
Axis III
In this Axis general medical conditions, which include brain injury and medical or physical disorders. These diseases or disorders may severely affect the patient’s mood or functioning. These disorders may aggravate other diseases or the symptoms of the present disorder.
Axis IV
The disorders in this Axis may be contributed by psychosocial and environmental problems. These include conditions influence the diagnosis, treatment, or prognosis of the mental disorder.
Here are some problems categories as listed by DSM-IV-TR: social environment problems, family problems, economic problems, educational problems, occupational problems, housing problems, health care access problems and war or disaster problems.
Axis V
In this axis it helps the doctor to rate the patient general level of functioning, which assists in creating a treatment plan. There two major scales namely the Children’s Global Assessment Scale and the Global assessment of functioning scale. The Global assessment of functioning (GAF) Scale is categorized into three scales, such as the Social and Occupational Functioning Assessment Scale (SOFAS), the Global Assessment of Relational Functioning (GARF) Scale and the Defensive Functioning Scale.
